Professional Facial Extractions: What They Can and Cannot Do for Clogged Pores
If you have ever stood two inches from the bathroom mirror and decided that one blackhead absolutely had to come out right now, this article is for you.
I understand the temptation. Clogged pores are annoying, and squeezing them can feel incredibly satisfying for about three seconds. Unfortunately, skin does not always reward determination. What begins as a tiny clogged pore can quickly become red, swollen, scabbed, or noticeably larger after a little too much enthusiasm.
After more than 30 years of working with skin, I can tell you that professional extractions can make a huge difference for the right type of congestion. The important part of that sentence is the right type.
Not every bump should be extracted. More pressure does not mean a better result, and extractions alone will not permanently prevent clogged pores from returning.
So, what do professional facial extractions actually do? Let’s clear it up.
What Is a Clogged Pore?
A clogged pore forms when oil, also called sebum, and dead skin cells collect inside a hair follicle instead of moving normally to the skin’s surface.
That buildup can appear in a few different ways:
- Blackheads, or open comedones, occur when the pore remains open and the material inside is exposed to air. The dark color comes from oxidation. It is not dirt, and no amount of aggressive scrubbing is going to make your skin “cleaner.”
- Whiteheads, or closed comedones, form when the opening is covered by a thin layer of skin. They often look like small white or flesh-colored bumps.
- Sebaceous filaments are normal structures that help move oil to the surface of the skin. They are especially noticeable on the nose and often refill after being emptied. They are not the same thing as blackheads, even though social media seems determined to treat them like a personal failure.
Congestion can be influenced by oil production, hormones, skin-cell buildup, genetics, certain skincare or hair products, and a routine that is either not doing enough or doing far too much.
What Professional Facial Extractions Can Do
Professional extractions are the careful physical removal of suitable surface-level clogs by a trained skincare professional.
When performed correctly, they can:
- Remove existing blackheads and some closed comedones
- Improve the look and feel of rough, congested skin
- Make clogged pores appear less noticeable once the buildup is removed
- Help clear areas that are not responding to home care alone
- Give us a much better view of what your skin actually needs next
There is an immediate benefit to clearing a pore that is ready to be cleared. Your skin may look smoother, feel cleaner, and have less visible congestion after treatment.
The goal, however, is not to empty every pore on your face. A good extraction session is selective. We remove what can be removed safely and leave alone anything that is inflamed, too deep, or simply not ready.
Skin is living tissue, not a sheet of bubble wrap.
What Extractions Cannot Do
Extractions are useful, but they are not magic. They cannot:
- Permanently shrink your pores
- Change your genetics or hormone levels
- Stop your skin from producing oil
- Cure acne by themselves
- Safely remove every bump in one appointment
- Treat deep cysts or painful nodules
- Replace a consistent home-care routine or medical acne treatment when it is needed
Clearing a clogged pore removes the current blockage. It does not switch off the conditions that allowed it to form. That is why some blackheads and whiteheads eventually return, particularly when oil production and skin-cell buildup are ongoing.

Why Professional Extractions Are Different from Picking at Home
I know what some of you are thinking: “But I am very careful.”
Client under bright, even magnifying lamp for clear, shadow-free extractions.
Maybe. You are also leaning over a sink, working under bathroom lighting, and trying to extract your own face from an angle no human neck was designed to maintain.
Professional extractions begin with properly prepared skin. At DERMASPACE, we cleanse and gently exfoliate the skin, then use our moisture pack and infrared warmth to soften congestion before we begin. Proper lighting, clean technique, controlled pressure, and knowing when to stop all matter.
That last one is especially important.
If a clog does not release with appropriate preparation and gentle technique, forcing it can push material deeper, increase inflammation, break the skin, and raise the risk of discoloration or scarring. The American Academy of Dermatology also warns that popping blemishes at home can push contents deeper and lead to more noticeable acne, infection, pain, or scarring.
Sometimes the most skilled extraction is the one we decide not to do.
What We Extract and What We Leave Alone
Blackheads and certain noninflamed whiteheads are usually the best candidates for professional extractions. Even then, we evaluate each area individually.
We are much more cautious with:
- Red, swollen, or actively inflamed blemishes
- Deep, painful bumps beneath the skin
- Cysts and nodules
- Skin that is irritated, sunburned, over-exfoliated, or otherwise compromised
- Bumps that may be milia or something other than acne
Deep or painful acne is not something to squeeze into submission. Cystic or nodular acne often needs a dermatologist and a medical treatment plan. Referring you for medical care does not mean we are giving up on your skin. It means we know when a concern needs a different level of treatment.
How We Approach Extractions at DERMASPACE
Our special facial wrap, soaked in nutrient-rich ingredients, is applied after extractions to soothe and heal the skin.
At DERMASPACE, extractions are an important part of our signature iDerm Deep Ionization Facial, not a rushed add-on at the end of a fluffy spa treatment.
We start with a thorough cleanse and gentle exfoliation. Next comes our moisture pack with yucca extract and infrared warmth, which helps soften stubborn congestion and prepare the skin. Once the skin is ready, we carefully remove appropriate blackheads, whiteheads, and other surface-level buildup.
After extractions, we move into the signature “Wrap & Zap” portion of the iDerm Facial. A gentle galvanic current helps deliver a customized solution containing water-soluble vitamin C, zinc sulfate, and other skin-supporting ingredients into the superficial layers of the skin. This step is designed to calm inflammation, support healing, and help restore balance after the deeper cleansing portion of the treatment. This is what makes us different than any other facial out there!
This is one reason the iDerm Facial is such a strong choice for congested and acne-prone skin. We are not simply squeezing things and sending you on your way. We clean, treat, support, and then make a plan for maintaining the results.
Also - regular treatments have proven to improve the issue in general which leads to less extractions even required.
Will Your Skin Be Red After Extractions?
Some temporary pinkness or redness is normal, especially if you had a lot of congestion. How long it lasts depends on your skin sensitivity, the amount of extraction work performed, and how stubborn the clogs were.
Your skin should not look as though it has been in a bar fight.
We would rather leave a difficult clog for another visit than create unnecessary trauma trying to force it out in one appointment. After treatment, keep your routine gentle, avoid picking, and follow the specific home-care instructions your esthetician gives you.
How to Keep Clogged Pores from Coming Right Back
Professional extractions clear what is already there. Your home routine helps reduce how quickly new congestion develops.
For most people, that means:
Cleanse consistently, but gently
Use a cleanser appropriate for your skin and remove makeup before bed. Scrubbing harder does not clean deeper. It usually creates irritation, which gives you a second problem without solving the first one.
Use the right exfoliation
Ingredients such as salicylic acid can help manage oil and dead skin inside the pore, but stronger is not automatically better. The best formula and frequency depend on your skin’s sensitivity, current products, and other treatments.
Choose products carefully
Heavy skincare, makeup, sunscreen, and hair products can contribute to congestion for some people. “Non-comedogenic” is a useful starting point, although no label can guarantee that every formula will work for every face.
Support the skin barrier
Trying to dry out clogged pores often backfires. Skin still needs hydration, and an irritated barrier is not the route to clearer skin.
Be consistent
This is the least exciting advice and usually the most effective. A simple routine used regularly will outperform an aggressive seven-product attack that lasts four days.
For clients with ongoing mild to moderate congestion, we may recommend an initial series of four to six facials for congested and acne-prone skin, scheduled weekly or every other week. Once the skin is improving, many clients maintain their results with treatments every two to four weeks, depending on their skin and goals.
Anything much longer than six weeks can feel a bit like starting over. Skin loves consistency, even when the rest of us do not.
When Should You See a Dermatologist?
Professional facials can be very helpful for blackheads, whiteheads, and mild to moderate congestion. They are not a replacement for medical care.
Consider seeing a dermatologist if you have:
- Deep, painful cysts or nodules
- Widespread or rapidly worsening acne
- Acne that is leaving scars or persistent dark marks
- A sudden or unexplained change in your skin
- Breakouts that are not improving with consistent professional care and an appropriate home routine
Our goal is healthier skin, not keeping every concern inside our treatment room. Sometimes the best plan combines professional skincare with a dermatologist’s care.
The Bottom Line
Professional facial extractions can be incredibly effective for clearing suitable blackheads, whiteheads, and surface congestion. The key is proper skin preparation, controlled technique, and knowing which pores should be left alone.
Extractions can remove the clogs you have today. Long-term improvement comes from combining them with appropriate products, consistent treatments, and medical care when necessary.
If clogged pores, blackheads, or recurring congestion are making you crazy, the iDerm Deep Ionization Facial is a great place to start. We will assess your skin, clear what can be safely cleared, and help you build a realistic plan for keeping it healthier.

Frequently Asked Questions
Are blackheads caused by dirt?
No. Blackheads form when oil and dead skin cells clog a hair follicle that remains open at the surface. The dark color is caused by oxidation, not dirt. Aggressive scrubbing can irritate the skin without removing the deeper blockage.
Do extractions make pores larger?
Properly performed extractions do not permanently enlarge pores. A clogged pore may look larger because it is stretched by the material inside it. Once the buildup is removed, it can appear less noticeable, although your natural pore size is largely influenced by genetics, oil production, age, and skin condition.
Do professional facial extractions hurt?
You may feel pressure or brief discomfort, especially around the nose or with stubborn congestion. Extractions should not feel like a test of endurance. If something is not releasing safely, it is usually better to leave it alone and address it over time.
How long does redness last after extractions?
Mild temporary redness is common and varies by skin sensitivity and the amount of congestion removed. Your esthetician will give you specific aftercare guidance based on how your skin responds.
Can extractions get rid of acne?
Extractions can remove blackheads, whiteheads, and other suitable surface clogs, but they do not cure acne or address every cause. Hormones, oil production, genetics, inflammation, products, and skin-cell turnover can all play a role. Moderate, severe, cystic, or scarring acne may require a dermatologist.
How often should I get facial extractions?
It depends on your skin. Clients with ongoing mild to moderate congestion may benefit from four to six treatments scheduled weekly or every other week, followed by maintenance every two to four weeks. We personalize the schedule rather than extracting skin simply because the calendar says it is time.
Can I extract blackheads at home?
It is safer to leave extractions to a qualified professional. Squeezing at home can push material deeper, increase inflammation, break the skin, and raise the risk of infection, discoloration, or scarring.
Which DERMASPACE facial includes extractions?
The full iDerm Deep Ionization Facial includes extractions and is our recommended starting point for most new clients with blackheads, whiteheads, or clogged pores. We also include extraction in our Advanced LED Facial and they can be added on to our Oxygen Infusion Facial. The express Wrap & Zap treatment does not include extractions.
